LOSS PASSPORT

Issuance of passport in lieu of lost passport

• Complete FORM-A
• Complete FORM-D with one notarized photograph.
• Compete Memorandum
• Complete Report of Lost Passport
• 6 current photographs (light blue background; not more than 6 months old)
• Original national ID card. (Copy if original not available)
• Fee $92 for first time loss ordinary (14 work days) or Urgent $240 (5 work days); $185 for second time loss ordinary and $480 for urgent; paid in Cashier check/ US Postal Money Order to Consulate General of Pakistan. All fees are non-refundable.
• Copy of lost passport
• Police Report (original)
• Documentary evidence showing proof of Pakistan nationality, i.e. citizenship certificate, Domicile certificate issued by the Government of Pakistan.
• Original Pakistan national identity card CNIC/POC/NICOP.
• If application is forwarded by mail, a notarized Affidavit with one notarized photograph on it should be submitted.
• The use of Express Mail is recommended for all deliveries. Self addressed prepaid envelope should be included if sending by mail. (USE USPS Express Mail ONLY for urgent items. We DO RECEIVE envelopes through FedEx, UPS, DHL and Airborne Express, but we CANNOT SEND return envelopes through them. Kindly ensure that return envelopes are of USPS only). Kindly note that the Consulate shall not be responsible for loss of documents sent by mail.
