Please note that the Consulate of Pakistan Chicago DOES NOT make NICOP or Any other form of ID Cards

NICOP/CNIC/POC are made in Pakistan, but overseas Pakistanis can send applications through Missions abroad. In the US the following offices process NICOP/CNIC/POC applications (also has links to downloadable forms).

Before downloading the form, please check which Consulate covers your State and send your application directly to them.
